Family First Program
For: Family caregivers of DOL White Card clients
Summary: Get trained, supported, and paid to care for a loved one at home. This RN-guided program includes caregiver education, monthly RN visits, and referral to an approved payroll partner.
Stroke Recovery Support
For: Individuals recovering from stroke or TIA
Summary: Our stroke care team helps clients safely recover at home with RN oversight, medication management, mobility support, and family education to reduce hospital readmissions.
Chronic Illness Management
For: Clients with conditions like diabetes, heart failure, COPD, or kidney disease
Summary: We provide skilled and non-skilled support for chronic illness — including monitoring vitals, medication setup, diet education, and ongoing RN assessments.
Dementia & Memory Support Program
For: Individuals with Alzheimer’s, dementia, or cognitive decline
Summary: Our in-home dementia care offers structure, companionship, and safety — with specially trained caregivers and RN oversight to ease the journey for both client and family.
Companionship & Social Visits
For: Seniors or isolated individuals needing support
Summary: We offer warm, respectful visits to promote engagement, prevent loneliness, and provide light assistance with everyday activities.
