Lifeline Home Health provides VA Community Care–authorized home health and in-home care services for Veterans across Washington State, including the Tri-Cities, Yakima, Spokane, and surrounding rural communities. As an approved VA Community Care Network (CCN) provider, we deliver skilled nursing and supportive care services that help Veterans safely remain at home while receiving coordinated, high-quality care.

VA Community Care allows eligible Veterans to receive healthcare services from approved community providers when care cannot be provided quickly or conveniently at a VA facility.

Yes. Lifeline Home Health is an approved VA Community Care Network (CCN) provider and works directly with VA Medical Centers and Community Care coordinators.
